Orthobiologics mainly mesenchymal cells and platelet-rich plasma is a promising treatment option as it significantly improves surgical repairs in patients with knee meniscal tear.

A recent analysis published in the Journal PM & R enlightens about the approaches used for meniscal tear treatment. Meniscal tears are the most frequently occurring pathology of the knee which happens due to a shear force within the femur and tibia. The beginning of the meniscal tear treatment involved conventional measures like physical therapy with referral for operative treatment for mechanical or persistent indications. As a consequence, the partial meniscectomy is implemented more than any additional orthopaedic method.

As these current reviewed methods whenever possible stated its value for the risk of early osteoarthritis following meniscus excision and the structural integrity of the knee joint. Multiple factors such as the location of the tear and patient demographics showed a significant role in choosing a relevant management strategy. The physical therapy continues as first-line therapy for knee pain, secondary to meniscus tear and should be proceeded in the management of acute and chronic knee pain.
Moreover, numerous evidence explains that ageing patients with complex meniscus tears in the context of degenerative arthritis should not go through arthroscopic surgery. The patients who are healthy, young and have tears close to the more vascular periphery of the meniscus, the direct meniscus repair is the option, but it is not suitable for all the patients. The orthobiologics; mesenchymal stem cells or platelet-rich plasma presents promising outcomes as standalone treatments or in augmenting surgical repairs. However, more investigation is demanded to estimate the role of orthobiologics in meniscal tear management.
