A split-mouth clinical trial compared the clinical effectiveness of local anesthetics Mepivacaine 2% or Articaine 4% (both with epinephrine 1:100.000), utilizing various anesthetic procedures to execute lower third molar germectomy and to gauge the patients' emotions and discomfort throughout the surgery.
A total of 50 individuals (age ranged from 11-16 years) who needed germectomy of mandibular third molars were included. On one side, local anesthesia was given using the Articaine inoculated with plexus approach, and on the opposite side, it was administered using the inferior alveolar nerve block technique with Mepivacaine.
Preoperative and intraoperative tactile-pressure sensations, as well as intraoperative pain measured using the Visual Analogue Scale (VAS) at four levels, were evaluated for the patients.
When Articaine was utilized, surgical procedures lasted less and with more effective analgesia. The Mepivacaine group mostly needed the additional intraosseous injection intraoperatively. Only a few individuals experienced tactile pressure. In 90% of the cases with Articaine, intraoperative pain sensation was absent. Substantial variations were noted that favored the usage of Articaine in the instances that reported "absent" and "moderate" VAS levels.
Articaine administered using a plexus anesthetic approach appears to be more clinically manageable than Mepivacaine for germectomy. The application of the Articaine anesthetic method reduced the discomfort of tactile-pressure emotions and pain.
