Background

A randomized split-mouth study aimed to assess effectiveness of nimesulide and nimesulide + thiocolchicoside combination following impacted mandibular third molar surgery.

Method

Overall, 35 patients undergoing extractions of two mandibular third molars, in mesioangular positions were segregated into 2 groups as per the medication given. Participants were administered 100 mg nimesulide + 8 mg thiocolchicoside after first surgery. For 15 days, the healing period was waited and in pursuit of the second surgery, only 100 mg of nimesulide was given every twelve hours for seven days.

For evaluation of pain in 6th, 8th, 12th, 24th, and 48th hrs and on 3rd, 5th, and 7th days post-surgery, visual analog scales (VAS) was used. Using digital calipers, mouth opening capacity before and after surgery on the 2nd and 7th days was measured (in mm).

Result

Compared to nimesulide, nimesulide + thiocolchicoside showed superior efficacy in terms of pain reduction. At 6th, 8th, 12th, 24th, and 48th hrs and on 3rd and 5th days, VAS levels of nimesulide + thiocolchicoside were reduced than nimesulide group. Contrasted to nimesulide group, higher mouth opening (trismus) was witnessed in nimesulide + thiocolchicoside group.

The trismus measurements were lower than preoperative measurements at end of the 7th day in the nimesulide group. No statistically significant difference was seen in thiocolchicoside + nimesulide group in preoperatively-7th day.

Conclusion

Compared to only 100 mg nimesulide, the combination of 100 mg nimesulide and 8 mg thiocolchicoside when administered orally has greater analgesic effectiveness and better trismus outcomes after mandibular third molar surgeries.
