A minimally invasive second-generation endometrial ablation is a new alternative to manage treatment-resistant menorrhagia or dysmenorrhea. 

As per new findings, postoperative administration of dienogest along with microwave endometrial ablation helps to alleviate symptomatic adenomyosis. Adenomyosis affects about 5 to 70 % of the population and manifests with issues like menorrhagia and dysmenorrhea. All these together leads to reduce female's quality of life. To manage this severe problem, Microwave endometrial ablation (MEA) is the treatment of choice. The MEA involves womb ablation to stop complications like menorrhagia. Also, a drug Dienogest is used to decrease dysmenorrhea by terminating endometrium. But some patients show resistance against dienogest.

Kuniaki Ota and colleagues conducted a trial to analyse the effect of a combination,  microwave endometrial ablation along with postoperative Dienogest therapy in adenomyosis and related symptoms management. A total of ten patients who were resistant to hormone treatment went through MEA and received oral Dienogest after surgery. The anaemia and pain recurrence was taken as primary endpoints. Change in adenomyosis and related symptoms recurrence were considered as secondary endpoints.

The haemoglobin levels and visual analog scale (VAS) scores improved significantly after the treatment. A statistically significant change was also noticed in myometrial thickness after the surgery. No symptomatic recurrence was seen. The results reflect that the combined treatment shows a considerable relief from both pain and bleeding.
